Dear Potential Volunteer,

Thank you for your interest in becoming a volunteer for Beats & Rhythms.

Enclosed are the forms you need to complete:

1. Volunteer application (2 sides),  complete with signatures
2. Liability/Indemnity release
3. Photo release
4. Permission for background check, with signatures

5. Copy of driver’s license or state issued ID

6. Copy of certificates or licenses (ie, RN, Lifeguard, CPR, MA)
